Presentations feature real-world experience and clinical evidence in lower-risk MDS, underscoring growing global scientific engagement with imetelstat

FOSTER CITY, Calif., Oct. 12, 2026 (GLOBE NEWSWIRE) -- Geron Corporation (Nasdaq: GERN), a hematology-focused biopharmaceutical company pursuing more meaningful moments for people living with blood cancer, today announced two oral presentations and one poster presentation highlighting imetelstat in patients with lower-risk myelodysplastic syndromes (LR-MDS) at the 88th Annual Meeting of the Japanese Society of Hematology and the 2026 Annual Meeting of the German, Austrian and Swiss Societies for Hematology and Medical Oncology. These presentations will feature real-world evidence and findings from the Phase 3 IMerge trial.

“We’re encouraged by the growing interest in imetelstat across the global hematology community, supported by both the clinical evidence from IMerge and an expanding body of real-world evidence in lower-risk MDS,” said Joseph E. Eid, M.D., Executive Vice President, Research and Development and Chief Medical Officer of Geron. “These presentations provide an important opportunity to share evidence relevant to patient populations across regions and engage with clinicians as we evaluate opportunities to bring imetelstat’s potential to more people living with LR-MDS around the world.”

Key takeaways from the presentations:

88th Annual Meeting of the Japanese Society of Hematology 2026

Real-World Experience With Imetelstat in Lower-Risk Myelodysplastic Syndromes: Insights From United States Claims Data (Eid, et al. Oral #O1-11C-4) The retrospective study used closed medical and pharmacy claims with linked enrollment and laboratory data from the Komodo Health database and included adults with MDS who initiated imetelstat between June 2024 and August 2025, representing the first 14 months after the launch of imetelstat in the U.S.In this real-world U.S. cohort (N=58), with a mean age of 75 years at diagnosis, most patients (71%) initiated imetelstat in the third line or later, with considerable anemia and a mean Charleson Comorbidity Index of 2.6 at the time of initiation.Despite substantial comorbidity burden, most patients remained on therapy through the early months when cytopenias typically emerge (~67% at 6 months; Kaplan-Meier [KM] estimate). Durable Clinical Outcomes With Imetelstat in Korean Patients With LR-MDS (Eid, et al. Poster #P3-12-5) This post-hoc study explores characteristics of two Korean patients with LR-MDS who had over one year of red blood cell transfusion independence (RBC-TI) with imetelstat, and explores the relationship between cytopenias and treatment outcomes with imetelstat in 13 Korean patients from the Phase 2/3 and heart rate-corrected QT interval (QTc) substudy of IMerge. Both patients who achieved long-term response with imetelstat experienced grade 3/4 neutropenia and grade 3/4 thrombocytopenia events early during treatment; most resolved to grade ≤2 in <4 weeks, consistent with the broader IMerge data.Durable efficacy and manageable safety with imetelstat was exhibited in Korean patients across a range of patient and disease characteristics (sex, RS status, baseline erythropoietin [EPO], and baseline platelet levels). In the 13 Korean patients, early cytopenias with imetelstat were associated with better clinical outcomes, as previously reported in IMerge. 2026 Annual Meeting of the German, Austrian and Swiss Societies for Hematology and Medical Oncology

Telomerase Inhibition With Imetelstat in Patients With Lower-Risk Myelodysplastic Syndromes: Integrating Molecular Findings, Cytopenias, and Clinical Outcomes (Brauer, et al. Oral #V755) This presentation integrates key evidence from the Phase 2/3 and QTc substudy of IMerge to support clinical practice, reporting on molecular changes, cytopenias and clinical outcomes with imetelstat in LR-MDS, including emerging evidence suggesting treatment may potentially alter the underlying biology of the disease.Transient cytopenias observed early in treatment may be the result of an on-target effect of imetelstat and may be predictive of a better therapeutic response.Certain patients with LR-MDS and transfusion-dependent (TD) anemia who experience sustained RBC-TI (≥8-week, ≥24-week, or ≥1-year) with imetelstat may experience improved health-related outcomes, including fatigue, QUALMS Total, QUALMS-P, and QUALMS-E scores. The presentations are available on the “Publications & Presentations” page of the Geron website.

About RYTELO (imetelstat)
RYTELO (imetelstat) is an oligonucleotide telomerase inhibitor approved in the U.S. for the treatment of adult patients with lower-risk myelodysplastic syndromes (LR-MDS) with transfusion-dependent anemia requiring four or more red blood cell units over eight weeks who have not responded to or have lost response to or are ineligible for erythropoiesis-stimulating agents (ESAs). It is indicated to be administered as an intravenous infusion over two hours every four weeks.

In addition, RYTELO is approved in the European Union as a monotherapy for the treatment of adult patients with transfusion-dependent anemia due to very low, low or intermediate risk myelodysplastic syndromes without an isolated deletion 5q cytogenetic (non-del 5q) abnormality and who had an unsatisfactory response to or are ineligible for erythropoietin-based therapy.

RYTELO is a first-in-class treatment that works by inhibiting telomerase enzymatic activity. Telomeres are protective caps at the end of chromosomes that naturally shorten each time a cell divides. In LR-MDS, abnormal bone marrow cells often express the enzyme telomerase, which rebuilds those telomeres, allowing for uncontrolled cell division. Developed and exclusively owned by Geron, RYTELO is the first and only telomerase inhibitor approved by the U.S. Food and Drug Administration and the European Commission.

Geron Corporation: FDA Approval Fuels Stock Price SurgeGeron NASDAQ: GERN executives said the company remains on track to generate 2026 RYTELO net revenue of $220 million to $240 million, with Chief Executive Officer Harout Semerjian saying the company now expects results toward the midpoint to high end of that range, or approximately $230 million to $240 million.

Speaking at the H.C. Wainwright 28th Annual Global Investment Conference, Semerjian said RYTELO, or imetelstat, was launched in July 2024 as the first telomerase inhibitor for patients with lower-risk myelodysplastic syndromes, or MDS. He described the therapy's mechanism as one designed to target the disease itself rather than mask symptoms.

Get Geron alerts:

Geron Stock Doubles After Imetelstat Receives FDA Panel Approval The company reported $57.5 million in net revenue in its latest quarter, which Semerjian called its highest quarterly sales figure since the launch. Geron has delivered three consecutive quarters of growth, he said, while acknowledging that the company still has work to do to expand use of the drug.

Commercial focus shifts toward community practices Semerjian said Geron’s commercial efforts are increasingly focused on the second-line lower-risk MDS population, particularly within community treatment settings. He said approximately 80% of the business in the disease area is in the community, making simplified physician messaging and engagement with high-volume community accounts important to the launch.

Geron initially concentrated on building experience with RYTELO at academic medical centers, according to Semerjian, partly because many clinical trials for the drug were conducted outside the United States. The company is now seeking to translate that experience into broader community adoption.

Chief Medical Officer Joseph Eid said the company has emphasized physician education on RYTELO’s mechanism, patient management and cytopenias associated with the new drug class. Geron has also expanded its educational efforts through continuing medical education programs and regional hematology meetings.

Eid noted that National Comprehensive Cancer Network guidelines have been updated twice since RYTELO’s launch and position the therapy as a second-line option. He also said luspatercept’s move from second-line to first-line treatment has supported RYTELO uptake, because some patients who receive luspatercept may have elevated serum erythropoietin levels that make erythropoiesis-stimulating agents less suitable.

Early-line mix and account additions Semerjian said 34% of Geron’s business, measured using a 12-month lookback, came from first- and second-line patients. The company views that as an important indicator because the IMerge trial and RYTELO’s approved indication were primarily focused on second-line patients.

He said physicians often initially use new oncology therapies in later-line, higher-need patients before utilization migrates toward the studied population. Geron expects its early-line mix to continue growing over time.

The company has added between 100 and 200 new accounts per quarter that had not previously ordered RYTELO, Semerjian said. While account breadth is especially important during the initial years of a product launch, Geron expects patient depth within existing accounts—particularly high-volume accounts—to become increasingly important over time.

Geron has not provided peak-sales guidance for RYTELO. However, Semerjian pointed to the size of the lower-risk MDS market and said luspatercept represents a business of roughly $2.5 billion and is growing in that setting. He also said Geron holds worldwide rights to RYTELO and has not entered into an ex-U.S. commercialization deal.

European strategy and myelofibrosis program Semerjian said Geron plans to provide an ex-U.S. strategy, particularly for Europe, before year-end. The company is evaluating multiple options but does not intend to pursue a traditional large-scale country-by-country commercial buildout, he said. Geron is considering payer dynamics and changes affecting global launch sequencing while holding discussions with stakeholders outside the U.S.

Geron also discussed its IMpactMF Phase 3 trial in relapsed or refractory myelofibrosis following treatment with a JAK inhibitor. Eid said the 327-patient study completed enrollment last September and compares imetelstat with best available therapy, with overall survival as the primary endpoint.

The company has been discussing the planned interim analysis with U.S. and European regulators to seek alignment on parameters that could support a positive outcome if the analysis is triggered. Eid said Geron’s base case has been that the trial proceeds to final analysis, noting that interim analyses for overall-survival studies generally present a high bar.

Semerjian said Geron had more than $300 million in cash and recently hired its first chief business officer. He added that the company reiterated operating-expense guidance of $230 million to $240 million, alongside its projected $230 million to $240 million in 2026 RYTELO revenue.

Geron Corporation: FDA Approval Fuels Stock Price SurgeGeron NASDAQ: GERN said it expects full-year net revenue from RYTELO to land in the mid-to-high end of its previously stated range, or approximately $230 million to $240 million, as the company continues to expand use of the telomerase inhibitor in lower-risk myelodysplastic syndromes, or MDS.

During the Cantor Healthcare Conference, Chief Executive Officer Harout Semerjian said Geron generated $57.5 million in net revenue during the second quarter, nearly $5 million above the preceding quarter. He said the company has now recorded three consecutive quarters of growth and reported that RYTELO revenue rose 24% in the first half of the year while operating expenses declined 4%.

Get Geron alerts:

Geron Stock Doubles After Imetelstat Receives FDA Panel Approval “We definitely see a path to profitability,” Semerjian said, noting that Geron’s revenue outlook and operating-expense guidance are both in the $230 million to $240 million range. However, he said the company is also focused on building a broader business and maintaining financial flexibility for future opportunities.

Community Expansion and Earlier-Line Use Semerjian said Geron’s commercial strategy has progressed from early engagement with academic medical centers toward greater penetration of community treatment settings. He said roughly 80% of lower-risk MDS patients are seen and treated in the community, particularly those in the second-line population targeted by RYTELO.

Geron is focusing on high-volume community accounts and on increasing use within accounts that have already adopted the treatment. Semerjian characterized this as a shift toward “depth” of use while the company continues to add new accounts.

The company is also seeking to move utilization toward earlier treatment lines. Semerjian said more than one-third of patients receiving RYTELO are now coming from earlier lines of therapy. He said Geron’s label and recently updated NCCN guidelines support use in the second-line setting, where the company believes its data are strongest and treatment duration may be longer than in later-line use.

While physicians may continue to use the therapy organically in later-line patients, Semerjian said Geron’s commercial efforts will center on second-line treatment.

European Strategy Under Review Geron has global rights to RYTELO and has received European Medicines Agency approval. Semerjian said the company plans to update investors by the end of the year on its strategy for markets outside the United States, including Europe.

Potential approaches could include partnerships, targeted commercial activity or use of contract sales organizations, he said. Geron does not plan to pursue a traditional model of building large country-by-country commercial teams in Europe.

Semerjian said the company is weighing pricing and policy considerations, including the evolving most-favored-nation, or MFN, discussion. He said management intends to preserve its flexibility and avoid making decisions that could undermine the company financially while pursuing broader patient access.

“RYTELO needs to be everywhere in the world, helping patients around the world,” Semerjian said. “We don’t necessarily need to be everywhere in the world.”

Myelofibrosis Study Discussions Continue Chief Medical Officer Joe Eid provided an update on Geron’s relapsed/refractory myelofibrosis study in patients who have failed JAK inhibitors. The trial, which completed enrollment in September of last year, uses overall survival as its primary endpoint.

Eid said Geron has been discussing the interim analysis with U.S. and European regulators to ensure the study’s framework reflects current regulatory expectations. The company is evaluating whether an interim result could potentially support a supplemental new drug application, though its previously communicated base case remains a final analysis in the second half of 2028.

The study could continue unchanged, reach a futility outcome or produce a positive result, Eid said. He added that Geron intends to complete regulatory discussions before any unblinding trigger is reached in order to preserve trial integrity.

The study permits certain control-arm patients to cross over to active treatment after a washout period and company review. Eid said the trial was designed with a buffer intended to prevent crossover from diluting the study’s statistical power.

Business Development and Capital Position Semerjian said Geron had more than $320 million in cash and has not exercised the final two tranches of a $125 million Pharmakon debt facility. The company recently added a chief business officer and is building capabilities to evaluate potential partnerships and business-development opportunities.

While RYTELO growth remains Geron’s top priority, Semerjian said the company may consider assets that fit its existing commercial and medical infrastructure. He said Geron does not need multibillion-dollar peak-sales assets to support its strategy and believes its available capital provides flexibility if attractive opportunities emerge.

Geron Corporation: FDA Approval Fuels Stock Price SurgeGeron NASDAQ: GERN reported second-quarter net revenue of $57.5 million, up 17% from a year earlier and 11% sequentially, as the company continued to expand use of RYTELO among patients with lower-risk myelodysplastic syndromes, or MDS.

Chief Executive Officer Harout Semerjian said first-half net revenue rose approximately 24% from the same period in 2025, while total operating expenses declined 4%. The company ended the quarter with $327 million in cash, cash equivalents, restricted cash and marketable securities.

Get Geron alerts:

Geron Stock Doubles After Imetelstat Receives FDA Panel Approval “We delivered another quarter of net revenue growth, expanding RYTELO's reach to more eligible patients,” Semerjian said. “We delivered another quarter of net revenue growth, expanding RYTELO's reach to more eligible patients, strengthened the clinical evidence supporting RYTELO, continued investing in future growth opportunities, all while remaining financially disciplined.”

RYTELO Demand and Commercial Execution Geron said RYTELO demand increased 5% in the second quarter compared with the first quarter, marking its third consecutive quarter of demand growth. The number of prescribing accounts increased 8% to approximately 1,575 since the product's launch.

Ahmed ElNawawi, Geron’s chief commercial officer, said patient starts in the first- and second-line settings represented 34% on a rolling 12-month basis. The company is prioritizing high-volume community treatment centers, earlier identification of eligible patients, account management and targeted engagement with health-care professionals.

Geron estimates that approximately 8,000 patients in the United States may be eligible for RYTELO in the second-line, lower-risk MDS setting.

During the question-and-answer session, Semerjian said sales growth is coming from both new and existing prescribing accounts. ElNawawi added that the company expects expansion into new accounts, or breadth, to become a smaller contributor during the second half, while greater use within existing accounts, or depth, becomes a more important metric.

ElNawawi said Geron does not currently have a reliable metric for measuring duration of therapy and therefore cannot determine whether persistence is increasing or decreasing.

Guidance Points to Higher End of Revenue Range Based on first-half performance, Geron expects 2026 RYTELO net product revenue to land at the mid-to-high end of its previously stated $220 million to $240 million guidance range. The company continues to expect total operating expenses of $230 million to $240 million for the year.

Chief Financial Officer Michelle Robertson said the company expects consistent quarter-over-quarter revenue growth through the remainder of 2026. Geron manages inventory within a two- to four-week range and does not expect significant inventory-related increases or decreases in the second half, she said.

Second-quarter gross-to-net deductions rose to 20.7%, compared with 15.3% in the year-earlier period. Geron continues to expect gross-to-net deductions in the low-to-mid-20% range for the remainder of the year.

Research and development expense was $22 million, compared with $21.7 million a year earlier. Selling, general and administrative expense was $38.9 million, compared with $38.6 million in the prior-year quarter. Total operating expenses, excluding cost of goods sold, were $60.7 million, compared with $60.3 million a year earlier. Robertson attributed higher research and development spending to investments in chemistry, manufacturing and controls, or CMC, and said higher marketing expenses affected selling, general and administrative costs. These increases were partially offset by lower personnel costs following the company’s workforce reduction in December 2025.

Real-World Evidence and Myelofibrosis Program At the European Hematology Association congress, Geron presented retrospective results from an investigator-sponsored real-world study conducted with Moffitt Cancer Center. The two-part retrospective and prospective study is evaluating RYTELO’s safety and clinical efficacy in advanced, heavily transfusion-dependent lower-risk MDS patients, including patients with extensive prior therapies and prior luspatercept failure.

Semerjian said the retrospective findings were generally consistent with the Phase III IMerge trial and supported RYTELO’s safety, efficacy and tolerability profile in a broader patient population. The data also indicated a trend toward better management of cytopenias and improved responses when RYTELO was used in the first three lines of therapy.

Chief Medical Officer Joseph Eid said the company expects to present insights from the prospective portion of the study at a future scientific meeting.

Geron also discussed its Phase III IMpactMF trial of imetelstat in relapsed or refractory myelofibrosis. The company has engaged regulators and outside experts regarding the design of the trial’s interim analysis and is evaluating a modification to the event threshold that would support registration if the data monitoring committee recommends unblinding for positive efficacy.

Eid said the potential modification concerns the interim analysis and would not change the final overall-survival analysis, which remains the trial’s primary endpoint. Geron’s base-case expectation remains for the final overall-survival analysis in the second half of 2028, while a positive result at the interim analysis would be an earlier upside scenario.

European Commercial Plans Expected Before Year-End Geron said it is exploring a “gated” commercialization strategy for RYTELO in Europe and other markets while seeking to preserve U.S. pricing integrity. The company expects to provide an update on its European commercialization plans before the end of 2026.

Semerjian said Geron sees a patient opportunity in Europe comparable in scale to the U.S. market and is engaging medical experts and payers. He said the company is monitoring evolving most-favored-nation dynamics and expects to assess potential approaches in major markets including Germany and France. Potential strategies could include partnership arrangements, he said.

Geron also announced that Chinmaya Rath joined the company as chief business officer. Semerjian said Rath’s appointment supports the company’s effort to identify strategic growth opportunities and build Geron into a leading hematology company.

About Geron (NASDAQ:GERN)Geron Corporation NASDAQ: GERN is a clinical-stage biotechnology company dedicated to developing and commercializing novel treatments that target telomerase, an enzyme critical to cancer cell immortality. The company's research is focused on hematologic malignancies and solid tumors, with a pipeline designed to address diseases that have historically had limited therapeutic options.
